Welcome to Animal Clinic of Chardon!
- We know that choosing the right veterinarian for your pet can be a challenging process. Your first visit to a new clinic can sometimes be stressful with wondering what to expect, forms to complete, and trusting new people to take care of your beloved pet. Please download our "Welcome Letter" to learn more about our practice.
- Our Dr.'s and support staff work together as a team to provide the best possible healthcare for your pet. We encourage you to download the "New Patient Registration Form" prior to your pet's appointment.
- If you are bringing your pet in for a Preventive Care Exam, please download our Preventive Care Questionnaire. If it is a Medical Exam, please download our Medical Questionnaire.

- If you have any medical history on your pet, please have it faxed or emailed to us prior to the appointment. This will help us in determining your pet's needs.

I was very blessed to have ACOC take care of my beloved lab, Gracie. She suffered from allergies for many years and as she became older had other health concerns. We become frequent visitors to the clinic and everyone always cared for her and loved her like I did. Recently, Gracie passed away unexpectedly. My heart has been torn, but even with her passing, the staff at ACOC has been supportive and caring. I recently received a beautiful keepsake from ACOC that reminds me daily of the wonderful times I had with her. I am so grateful to the staff for the care, affection and support that was always shown to Gracie and I (my son also). Without ACOC, Gracie last years wouldn't have been as peaceful as they were. Moved to Mayfield Village & started going to other vets. What a mistake! Making the drive back to Chardon now with my 6 cats and 1 dog because The Animal Clinic is the best!

Heartworm Prevention
Heartworm disease is a severe and potentially fatal disease caused by parasitic worms. Heartworms are transmitted by mosquitoes. The worms live in the heart and arteries of the lungs primarily of dogs but can also infect cats and other mammals. Heartworms can easily be prevented by using a monthly preventative.
