At The Animal Clinic of Chardon, we are proud to offer International Health Certificates and Hawaii Travel Pass services for your pet. Whether you’re planning an overseas adventure or a trip to Hawaii, we’re here to help you navigate the travel process with confidence and care.
A USDA-accredited veterinarian is required to complete the necessary documentation to ensure your pet meets all travel and destination-specific requirements. Regulations vary by country and can be complex, so we recommend contacting us as soon as you know your travel destination and approximate departure date. Start the Process To begin, please complete the appropriate JotForm below based on your destination. Once submitted, our team will review your information and contact you to initiate the certification process. We’ve also included downloadable informational packets to help you understand the steps, timelines, estimated costs, and re-entry requirements for the United States. Please note: A non-refundable deposit of $150 is required to begin preparing an International Health Certificate. This fee covers:
Once your documentation is reviewed, our veterinarian will contact you (via email and/or phone) to discuss any required testing, certifications, or vaccinations, and to schedule the final exam before travel. Updated estimates will be provided as needed. |

European Union / United Kingdom
Includes: Austria, Belgium, Bulgaria, Croatia, Cyprus, Czech Republic, Denmark, Estonia, Finland, France, Germany, Greece, Hungary, Ireland, Italy, Latvia, Lithuania, Luxembourg, Malta, Netherlands, Poland, Portugal, Romania, Slovakia, Slovenia, Spain, and Sweden. Note: The UK (England, Scotland, Wales, Northern Ireland) is not part of the EU, but follows similar travel guidelines. Basic International Health Certificate For all other international destinations (not EU/UK) Hawaii Travel Strict regulations apply for pets traveling to Hawaii, including quarantine guidelines.
