If you have not yet made an appointment, please call 440-285-9191 or download our app. Once your appointment is made, we ask that you complete our Client Registration Form. Please submit your pet's prior medical records at least 48 hours prior to the appointment. This will give our staff ample time to review.
Please choose from the following options:
- Submit them with our Send Medical Records Form.
- Take a photo of the records and attach to the chat feature on our app.
- Email them to info@acocvet.com
- Fax them to 440-285-3491.
- At least 24 hours prior to your appointment, please fill out our Pre-Visit Form.
